Immunization

The state of Minnesota requires by law all students enrolled in grades pre-K through 12 to show they have received immunizations, or to provide a signed exemption form. For a list of required immunizations, check out the link below or contact the District School Nurse.

Records can be emailed, faxed, or dropped off at your child's school.

For Kindergarten, your child needs:

DTaP: 5 doses

Polio: 4 doses

Hepatitis B: 3 doses

MMR: 2 doses

Varicella: 2 doses (or if history of chickenpox, submit Month/Year of illness)

 

For 7th grade, your child needs: (in addition to previous immunizations)

Tdap: 1 dose

Meningococcal: 1 dose + a booster at age 16
